How to Complete a New Form

Electronic Form

  • Complete the form;
  • Click on "Submit" button at the bottom of the page;
  • Form will be electronically sent to the appropriate recipient.

Automated Word Form

  • An Automated Word form will have "shaded boxes" in the response areas;
  • Open the form;
  • Do a "Save As" to your Desktop;
  • Open the form on your Desktop;
  • Complete the form and save it;
  • Open Microsoft Outlook;
  • Click on New Mail Message
  • Enter Recipient's name and Subject
  • Click on "Insert" on Toolbar > File
  • Desktop will open, Highlight your File;
  • Click on Insert, document will be inserted in your email;
  • Send to recipient.

Word Form

  • Print out the form, complete and mail it to the Recipient via "Inner Office" mail.
