  • Request to Change Name on Diploma

    In order to reissue a diploma or certificate with a different name, students must submit a Name Change Request Form and a Diploma Re-issue Request together with the required documentation to the Registrar's Office.

    Former Students:

    Contact the Registrars Office at 531-MCC-2353 or registrarsoffice@mccneb.edu to request a Name Change Form and for further assistance.  

    Current Students:

    1. Complete the Name Change Form electronically and attach appropriate documentation.

    2. Complete the Request to Reissue Diploma form and email form to registrarsoffice@mccneb.edu for processing once name change is complete.

    Important notice: Replacement diplomas will be noted REISSUE with the reissue date.
